Why 25 mg is not always a starting dose, how 90-to-900 tablet packs are supply not a protocol, and what to do when you cannot eat or drink.

A large bottle of 25 mg tablets is a convenience of postage, not a hint to take more than one a day. Licensed adult dosing is once daily. If a prescriber still has you on 10 mg, do not split our 25 mg tablets to invent a starter — ask for the strength that matches the script, or stay on the pack you were given locally.

Sick-day thinking

Vomiting, diarrhoea, fever, or a planned operation with a long fast raises the risk of ketoacidosis and volume depletion on SGLT2 drugs. Many diabetes teams tell patients to pause empagliflozin, sip fluids if allowed, and restart only when eating and drinking normally again — with ketone checks if you feel unwell. Copy that from your own clinic letter, not from this paragraph alone.

Weight is a side effect, not the licence

Calories lost in urine can trim a little weight. Empagliflozin is not a dedicated slimming tablet and is a poor substitute for a prescribed GLP-1 when weight is the main goal. Using it “off the diabetes plan” still carries genital infection and DKA risk.
