Oral semaglutide is the same GLP-1 medicine as the weekly injection, swallowed as a once-daily tablet. Absorption through the stomach is fragile. Food, coffee, a full glass of water or another pill taken at the same moment can cut how much drug actually gets into the blood — so a 14 mg tablet taken badly can under-perform a 3 mg tablet taken correctly.

The empty-stomach window
Licensed directions usually say: take the tablet first thing after an overnight fast, swallow it whole with a small sip of water (often no more than about 120 ml), and wait at least 30 minutes before eating, drinking or taking other oral medicines. Do not chew, split or crush it. The tablet uses an absorption enhancer; a large volume of liquid or a breakfast sandwich wrecks that design.
If you routinely take thyroid hormone, blood-pressure tablets or supplements at dawn, ask the prescriber which drug goes first. Many people shift the other tablets until after the wait, rather than stacking everything in one gulp.
Missed doses and travel days
If you remember the same morning and you are still fasting, take it and keep the wait. If you have already eaten, skip that day rather than doubling the next morning — unless your clinician has written a different rule. Crossing time zones is easier if you pick a local “first thing after waking” slot and stay consistent than if you chase the clock from home.
Tablets do not need the same fridge-first storage as unused pens, but they still hate humidity and heat. Keep blisters in the original pack, not a weekly pill box that sits on a sunny kitchen sill.
Why nausea still happens
Even with perfect swallowing technique, GLP-1 medicines slow the stomach. Smaller, lower-fat meals in the first weeks help. Persistent vomiting, signs of dehydration, severe abdominal pain or sudden vision change need urgent medical review, not another 14 mg tablet “to catch up.”
People who are pregnant, planning pregnancy or breastfeeding, or who have a personal or family history of medullary thyroid carcinoma or MEN2, are usually told to avoid this class. Say so before you order.
